Requirements for all patients requesting Virtual TeleMedicine (VTM) visit:

• Option 1: Video + Audio

  • Cell phone with camera and/or a desktop/laptop computer/tablet that has a camera in it for virtual medicine

  • Cell phone number for cell phone device and an email for desktop/laptop device

• Option 2: Audio Only (information gathered will be limited with this type of exam)

  • Cell phone

• Option 3: Email Only (information gathered will be limited with this type of exam)

  • Cell phone/Tablet/Laptop/Desktop

If you do not have either a phone with camera or laptop/desktop with camera, we recommend you schedule an In-Office visit.
